When I try to insert an alphabetical index and specify 2 columns, that choice
is ignored, and the index is generated with only 1 column. Then I can edit the
index and again specify 2 columns, and then the index is regenerated with 2
columns. It would be so much faster and simpler to generate it with 2 columns
to begin with.
