I'm going to close this one, since this is such a fundamental difference (and
odd case) between LO and Word.

We can't really throw away "Vocabulary" because it is part of endnote #1, but
LO has no ability to put random paragraphs before the marker number. So the
current implementation is pretty good considering that incompatibility.
