proposed changes to Format and Styles menus

1. Attached is proposal for Format menu "before" and "after".

2. Plus the "direct format lists" should be removed from Styles menu.

(UI reason: clear, consistent separation of Styles and direct formatting)
(UX reason: multiple comments in BZ about how users need to learn styles and
the difference between direct formatted lists and styled lists - so -- UI
should facilitate that ideal by not mixing these things up.)
