Compact, yes. But what should I check for Footer/Header on first page and
alternating on left/right? I mean the options are not so clear to me. Does "[ ]
Odd page" and [x] Even page (o) Same as odd page" mean off? 

If I'm wrong it might be solved with a better terminology. But I'd still add
some "preset" options for the most often used configurations like

(o) No Footer/Header
( ) Footer/Header equally on all pages
( ) User-define configuration
    [ ] Odd page header
    [ ] Even page header: (o) Same as odd page  ( ) Separate
    [ ] First page header: (o) Same as odd page  ( ) Separate
