Yeah, it is confusing, when having to write documentation for it.
"First in following list" is a "special" case. It is always added to the list
that comes prior.
"Second (or any other position) in following list" depends on which list was
selected last. That is, the numbering that is applied is determined by the list
where the cursor is located when the command is executed.
[this is the "background" or "reason" for that what is popularly called the
"directional" effect or "Easter egg" (though I am not certain that it is an
Easter egg). The "directional effect" focuses only on an accident of the
geometry of a 2-dimensional surface. I prefer to called it "last-selected", and
use the mnemonic "last-selected wins" -- except for this "special" case of
"first" in the list.]
