I tried recreating the DB and template using the actual spreadsheet. 
1. Saved spreadsheet under new name
2. Created DB over spreadsheet
3. Opened Test Doc
4. Selected File, New, Labels
5. Selected and Moved fields into setup and New Document
6. Moved fields into position adding text to some fields
7.  Synchronized fields
8. Saved file as xxx.ott
9 Selected Print, Yes and LO crashed:
