Could you explain in more detail what you mean by steps 5 and 6 ?

Your assumptions were correct. 
Step 5 I selected the fields from the registered database and moved them onto
the label form.  

Step 6 I ensured that positioning was correct by inserting new paragraph
characters and inserted text descriptions adjacent to some of the database
fields.
