Make a text with paragraphs in style "Text body" and headings in style "Heading
1". Go to menu Tools > Chapter Numbering (former Outline Numbering). Go to tab
"Numbering". Mark "1" in list "Level". Make sure "Heading 1" is chosen in
drop-down-list "Paragraph style". Select item "A, B, C" from drop-down-list
"Number". OK
Now your heading should be numbered with A, B, C.

When you use "Ctrl+Enter" a paragraph is inserted, which has the same paragraph
style as the paragraph has, where the cursor stood, when you have inserted the
page break. In this case, the new paragraph gets the paragraph style "Heading
1". And because this paragraph style is assigned to outline level 1 numbering,
it should get a number.
If you save the faulty document, you can see, that the new paragraph has got a
wrong paragraph style. It should have got a reference to "Heading 1". But it
has got a reference to a new automatic style and this automatic style has
erroneously got an attribute style:list-style-name="", which prevents
numbering.</pre>
