According to app.xml in <span class=""><a href="http://bugs.documentfoundation.org/attachment.cgi?id=127891" name="attach_127891" title="offending .docx file">attachment 127891</a> <a href="http://bugs.documentfoundation.org/attachment.cgi?id=127891&action=edit" title="offending .docx file">[details]</a></span>, the file was modified by LibreOffice
5.1.4.2 on Windows, but as it has <Template>Normal.dotm</Template>, it was
mostly likely created in MS Word, so it is likely that LO produced these errors
when saving.</pre>
