<pre>I eventually found the problematic setting well-hidden in paragraph formatting.
Problem is, I did not set this!
I just did normal editing of the document, and at some point, the TOC had the
additional text in it.
Something, somewhere, changed some paragraph formatting to "Outline level 4"...
