Can be an problem with your OpenGL driver, but since you disabled OpenGL and the problem persists, I doubt it. Please check that %appdata%\LibreOffice\4\cache\<icon theme name> doesn't exist (the Windows default theme name is collibre) and if it exists remove it. Broken icons won't disappear on their own. You have to remove the cached icon directory manually.
