On pc Debian x86-64 with master sources updated today, I could reproduce this.
However I noticed these logs:

Unzipping the odb file, I noticed in content.xml references to Obj82 and Obj41
for these forms and indeed the corresponding files weren't present in the odb
file.
I just removed the references from content.xml and rezip the whole structure.
1) Could you confirm the file I attached is ok?
2) Would you know a process to recreate a file from scratch with at least 1
form impossible to remove?</pre>
