<pre>It is apparent that for "Endnote" paages, the "Same Content on first page"
option (Page style > Header) is non-functional: i.e. it acts as if it is
permanently ticked. Therefore the header is defaulting to the "Same content on
left and right pages" option (Page style > Header).</pre>
