It is possible to add toolbars to documents (rather than Writer), and have
those toolbars remembered with the document. It is also possibile to associate
an icon with a toolbar entry (it works fine while the document is open).

When the document is closed an re-opened, the toolbar is remembered, but all
the items have lost their icons and reverted to text.

Steps to Reproduce:
1.Create a document in writer.
2.Add new toolbar to the document.
3.Add item to toolbar
4.Customize the item to use a standard icon
5.Check that it is displaying correctly.
6.Save and Close the document
7.Re-open the document
8.Note the missing icon

Actual Results:
Icons lost

Expected Results:
Icons should still be present
