Description:
Creating a simple form using the Form Wizard will not create the form after
complete specification.

Steps to Reproduce:
1. Create new DB with simple table with auto-generated primary key.
2. Create form using Form Wizard for all fields in single table except primary
key. See video for sequence.
3. After specifying form, click Finish. Nothing happens. Can not use form or
edit form. Cancel is the only way out.

Actual Results:
Form is not created. Need to exit Form Wizard to continue.

Expected Results:
Form to be created and either used to enter day or invoke form editor.

Reproducible: Always

User Profile Reset: No

Additional Info:
See video showing problem at:

Note another possible bug is show in the video where the Form Wizard seems to
want the auto-generated primary key on the form even though I did not select
it. I had to manually un-select it.
