Open the attached file. Note the Table of Contents at the beginning. This was
created by inserted the chapter page numbers using "Insert Cross-reference >
Cross-references" then selecting "Headings", "Page" and "Chapter 1/2/3/4/5".
The issue is that the page number format of "Chapter 1" is set to "i, ii, iii
etc" (Page style > Page > Page numbers). But, this has been entered as a
standard Arabic numeral in the Table of Contents.
FIX: Need to ensure that the page numbers of cross-references always display
the correct page number format as set in "Page style > Page > Page numbers".</pre>
