Distinct comment actions are on two places in Menu

* Insert > Comment
Consolidate or/and add Insert comment also to Edit > Comment submenu
Steps to Reproduce:
1. review a document and add comments and reply to comments
Actual Results:
The user has to visit two different parts of the menu tree to insert a comment
a to reply to an existing comment.
Expected Results:
Actions to comments shall be consolidated to a single place. Duplicates is not
a shame (the Insert > Comment can stay), but a reviewer can be troubled by
going to different places in the menu, when manipulating comments
