How about:
[ ] Odd (left?) page header
[ ] Even page header: (*) same as odd page ( ) separate
[ ] First page header: (*) same as odd page ( ) separate
Gives a very compact, intuitive, and complete coverage of options. The
radio-buttons could be comboboxes instead. When odd page header is deselected,
the others can only have "separate" kind.</pre>
