I have reworded the bug's title from "Form option buttons not reachable with
tab key" to "Form option buttons and other form fields not reachable with tab
key" since this accessibility issue appears to apply to all form fields.
As I mentioned in <a href="show_bug.cgi?id=108687#c34">comment #34</a>, this needs to be tested after saving the form in
read-only mode and reopening the document.
