I can confirm the second bug: name of items "Illustration" 
("Afbeeldingen" in Dutch) misses first character in dialog Insert > 
Fiels > Cross reference.
So it shows as "llustrations" / "fbeeldingen"

There is no problem with the reference to headings, bookmarks.

I remember someone has worked on styles / showing of numbering for 
Illustrations. Months ago :-) Or show it at a different position ..
Would not be surprised if that is somehow related...
