Laszlo Nemeth has fixed
Bug 37954 - EDITING Writer AutoSpellcheck flags capitalized words as
misspelled despite being added lowercase in the custom dictionary

Status: RESOLVED FIXED (available in v4.0).

So Urmas wrote in Bug 61167 - Suggestions for titlecase prohibited
words in user dictionary:
"This should be brought in line with the recent support for cased
suggestions with standard user dictionaries."

Exeption dictionaries are user-defined dictionaries as follows:
"Specifies whether you wish to avoid certain words in your documents.
In this way, you can create a custom dictionary of all the words to be
avoided. If this exception dictionary is activated, during
spellchecking you receive a corresponding note about any words which
should be avoided."
