Sorry, IMHO I don't see this as especially helpful GUI addition. 

The UI provides for tweaking things in a rational and deliberate fashion with
quick changes done with the "Visible Buttons" split menu, and more involved
changes via the Customize Toolbar--including the important ability to reset to
defaults.

Believe, the "Visible Buttons" being grouped with other toolbar controls and
positioned the same for all toolbars is intentional and brings consistency to
the UI.

While it might seem helpful, believe placing a configuration button alone at
the top of the menu would cause confusion and a poor UX. Placing a "Remove"
button so prominently on the context menu of all toolbars,  "What is receiving
the action of the remove?". 

Rather, things are more deliberate, Toolbars are stateful (retained in user
profile), and are intended to be "customized" to a users liking -- but with
guidance. 

The Visible Buttons manipulate just the default buttons normally associated
with the toolbar. While the Customize Toolbar offers more ambitious changes for
adding widgets and modifying toolbar defaults. And of course offers recovery by
"restoring" defaults without having to clobber ones profile.

Just don't see much benefit to implementing a Remove button, as things now are
pretty good.
