Well spotted. I updated the mockup
* changed 'Text Entry' to 'Heading Text'
* added the item 'User Text "<value>"'
* added the grouping title 'Hierarchy'
* changed 'Level' to 'Level in TOC' (for clarification and to fit the the text
length below; it would be better to have another group label and use 'Hierarchy
Level' here)
