Quick Mock-up

While I agree that a way to show where direct formatting is is highly
desirable, in my opinion the use of highlighting for that task is not a good
idea because that automated highlighting will be easily confused with a real
document formatting. Any system used to indicate where direct formatting is
applied must be easily recognizable. See attached image for an idea.
