Huge footnotes

Attempt to emulate Endnote page coverage with footnotes

Document is DIN A6 portrait so that pages are quickly saturated. Footnote
extends over several pages. Footnote area is "Not larger than page area" but
does not cover the whole page. a 2.26cm-high area is still reserved for main
text.
