Which .uno is being proposed here?

1. The "Settings" dialog (.uno:FootnoteDialog) (i.e., the one current found in
the Tools menu) would not make sense in the Insert menu (because nothing is
inserted).

2. Meanwhile the current "Footnote or Endnote" (.uno:InsertFootnoteDialog)
dialog in the Insert menu does not make so much sense.  afaict, in the standard
toolbar interface, its only value is to make a "custom" footnote anchor (which
it does not remember).  The other features in that dialog are useless, given
the context where it appears in the Insert menu. 

It might be better to strip down the dialog to just have the possibility to add
the anchor, and rename it "custom footnote/endnote" (where it also remembers
the last used choice).

But this .uno is also used in several of the notebook interfaces, and I do not
know how this proposed change would interact with those variations.
