I'll illustrate with a slightly v-longer row:

|      |      | 
|      |      |

| abcd |      |
|      |      | 
|      |      | 
|      |      | 
|      |      | 
 -------------

both of these are valigned to the top, but the first example has the meanlines
aligned, while the second example has the extenders aligned (it also happens to
have the baselines aligned but that's because it's hard to draw it differently
with ASCII art)
