#ifdef DBG_UTIL

lines enclosing declarations of variables/functions accessed in the
debug macros to

#if OSL_DEBUG_LEVEL > 0

(so the code is actually compilable :) and removed a bunch of now
unnecessary includes of tools/debug.hxx .
