binfilter was completely broken; if you remove stuff from tools please
build with --enable-binfilter.  i just spent the last 2 hours fixing
that by copying over ref.hxx and pstm.hxx from tools to binfilter, then
pulled and noticed that Caolan had fixed it already half an hour ago, in
a completely different way...
