Outline numbering gets restarted every time doc is opened

Attached file contains a doc w/ outline numbering applied to Heading 1. Every
time the doc is reopened (which I call a "session") and style 'Heading 1' is
applied to some text in the middle of the doc, then that heading gets no
number. If one reapplies outline numbering to 'Heading 1' then the outline
numbering of the text to which 'Heading 1' was applied in this session restarts
at 1.
