[Mesa-dev] [PATCH 0/4] KHR_debug clean ups and fixes

The main reason for this series is to remove the unneeded 
remapping of ARB_debug_output functions and just use an alias instead.
Testing shows this is what Nvidia and AMD do.
There is some other tidy ups and a bug fix too.

Passes existing KHR_debug and ARB_debug_output piglit tests and also
passes this unreviewed piglit test http://lists.freedesktop.org/archives/piglit/2014-February/009605.html
