What does you command line look like, and what version of meson do you have?

I can do:
$ CFLAGS=-Dfoobar CXXFLAGS=-Dfoobar meson build -Dbuildtype=debug
$ grep 'DDEBUG' build/build.ninja

and I see 2055 definitions of -DDEBUG
