Yeah I see this occasionally, well, the 2nd problem at least.  I have a theory
on what causes it but haven't had a chance to look at it yet.

I've attached something that should workaround the issue if I'm right about the
cause.  Let me know if it helps!

If it does, I'll look at backporting some of the patches from the -ng branch of
the ddx which'll be a better fix until -ng is usable.
