Here are the exact steps I have taken to achieve 3840x2160 with my NVIDIA
Corporation G96M [GeForce 9600M GT]:

Besides this I would believe this report to be very useful for anyone else who
would like to test for nouveau.hdmimhz and/or nouveau.duallink.
