I will try to back trace this problem as you suggested. I didn't meant 
"note -->insert--note"
It's menu item "insert-->note" also available as CrtlAlt+N.
If I do like you suggested
1. start oowriter
2. open a blank text document and write "hello word"
3 open dialog form main menu "Insert\Note"
4. write something in my note as "this is my note"
5. save it in Microsoft word doc format
6. oowriter is crashing.
