720x576 is not an NTSC resolution, you probably want 720x480. And no 
need/use to provide a modeline, it won't be used anyway. No need either 
to force the PanelSize, this is for built-in laptop screens. Some other 
options could be removed (SWCursor, EnableAGPDMA, ...), but this is 
unrelated to this very issue

You're not providing much information about the actual issue. It would  be
nice to know which version of the driver you are using, etc... A 
(compressed) xorg log would probably help.

Also, there is an outstanding bug with TV out in 0.2.904 and trunk. 
Please take a look at ticket 242. If this applies to you, I'd be
grateful if you can test the patch attached in comment 28 :
