Two loading fixes

I've patched two module loading fixes to p11-kit, and would appreciate
testing by anyone interested:

The first fixes a corner case where a non-critical PKCS#11 module init
fails, and was the last module in the registered list, which caused the
entire initialization process to erroneously fail:

And secondly, to prevent confusion, fail early when loading the
p11-kit-proxy.so module as a registered module itself. This leads to
loops and strange error messages:
