A couple of questions about this:

  - What driver version and hardware (for both AMD and NVIDIA)?  If I'm 
going to call someone out in a commit message, I like to be specific... 
in case later drivers fix the issue. :)

  - AMD crashes in glGenProgramPipelines, but they also do the wrong 
thing for glIsProgramPipeline (return GL_TRUE before the object has been 
bound)?

  - NVIDIA crashes in glGetProgramPipelineiv, but they also do the wrong 
thing for glIsProgramPipeline (return GL_TRUE before the object has been 
bound)?
