[Pm-utils] [patch commit] More minor cleanups

Done, although my rebase-fu was weaker than I thought, and I was
forced to reapply the 99video typo fix.

I also added a HOWTO.hooks file in the pm directory that gives a brief
overview of how we call hooks, what we expect them to return for exit
status codes, and what sourcing /usr/lib/pm-utils/functions does.
