a PDF that contains a choice field without item

How to reproduce:
1. open attached PDF that contains a choice field without item.
2. choice "Forms" from left "Demos" menu.
3. push "Get Forms Fields" button at the right top.
4. select "Choice" Form Field Type item in form list.
5. SEGV.
