A pdf file with a freetext comment having an Appearance stream

FreeText Annotations having an Appearance Stream (AS key in the dictionary) do
not display correctly. (Tested in okular, evince doesn't have this problem.)


FreeText annotations having an Appearance Stream but not having a DA key in the
dictionary do not display at all. (Both evince and okular.)

The second part formally follows the spec which says that the DA key is
required. However it also says that the AS key shall take precedence over 
the DA key so it would seem logical to use the Appearance Stream even if no
DA key is present.

I have attached an example file demonstrating the first problem.
