The difference is the thickness the line is rendered at. The test case
specifies a line thickness of 0.3985 points. Viewed at 100% on a 100dpi monitor
this results in a thickness of 0.3985*100/72 = 0.55 pixels.

There was a recent change to the cairo backend to ensure all lines are at least
1 pixel wide as required by the PDF specification. This was because of bug
reports where very narrow lines were almost invisible when displayed. If you
zoom in the line width will exceed 1 pixel on the screen and the smoothness
looks correct.
