changes to the qt4 frontend to use multi-thread by default

Might be useful if the changes to the internal API are implemented. Makes
Poppler::Page::renderToImage always create a new SplashOutputDev and enables
multi-threading by default. Also removes other code that becomes unnecessary
because of the changes.
