okular testcase

I removed the incRef and decRef again, but I haven't put the Document * in a
QSharedPointer (I'm not familiar enough with QT). And I changed the annotations
stuff, too.
BTW, I think it could be a nice idea to render the thumbnails in an extra
thread. This is possible now, too, but that's up to the okular developpers if
they agree.
