Enhancements for damaged PDFs

clang gave me a hint, unfortunately not the solution. And working on a solution
I had more than one moment where I wanted to give up or at least planned to
change the solution that it isn't allowed to reconstruct xref tables in the
thread based solution.
But finally I succeeded: the attached patch doesn't crash (or at least it seems
to be) anymore on broken PDF without getting new problems. I stopped my tests
either with the clang or with the normal compiled version after running 5
minutes without any crashes.
