Yep - I'm using Cairo 1.12.2 on Ubuntu 12.10, so that explains it. I'm
delighted that it's already fixed. 

Thank you everyone for your help; I was typesetting a book when I found the
problem and knowing I can work around it makes a huge difference to the
workflow.
