mupdf-x11 also renders all pages black from p.17 ff.

Ghostscript (tested with v9.10 and 9.11 git-prerelease) is able render pages
from p.17 ff "correctly".

The fact that MuPDF (which is a completely different PDF renderer
implementation from Poppler and/or Ghostscript) also has the same problem seems
to indicate that the file itself is not completely kosher.
