fix fillToStrokePathClip crash and rendering

This patch fixes this crash and fixes the rendering for this document and the
various dupes.  Removes the call to cairo_get_dash_count in
CairoOutputDev::fillToStrokePathClip.  It makes strokePathClip reference
counted because when drawing tiling patterns it may need to be kept around for
more than one drawing operation and uses fillToStrokePathClip in a few more
places.
