After few more tests, I had more trouble to display forms PDF with poppler

I tried to render a LaTeX's PDF with Evince using latest poppler version 0.54,
and radio button still not have the expected behavior, but attachments works
fine with chromium.

PDF file and LaTeX file in attachment.
