I'll attach a form (an official US gov tax form) that doesn't correctly render
in poppler/evince.

Particularly the "ticks" on the form boxes aren't rendered, also the background
color of the input fields is missing. I'll attach a screenshot with chrome
(rendering looks correct) versus evince.
